Provides for the expansion of primary healthcare services for a caseload of patients in the community.
Coordinates the medical plan of care with the nursing plan of care. Provides education and community resource referrals to patients and caregivers.
Evaluates the bio-psychosocial health and developmental history of the patient.
Performs basic physical assessment using observation, inspection, auscultation, palpation, and percussion.
Discriminates between normal, normal variations, and abnormal deviations by specific screening tests and refers patient with abnormal findings to appropriate members of the primary healthcare team.
Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date.
Licensed Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
Graduate of an accredited school of nursing or equvialent required
Three years of experience required.
