Purpose : As part of a multi-disciplinary Total Population Health Management team, optimizes outreach efforts focused on successful recruitment of PCORI 3D research study prospects. (The goal of the research study is to achieve better outcomes for cognitively vulnerable community-dwelling older adults and their families. Cognitive vulnerability means living with dementia, depression, and\/or delirium [the 3Ds]). Executes tasks associated with outreach to members and\/or caregivers of potential PCORI 3D Study participants, and referral to UConn Center on Aging staff (e.g., Research Assistants).\u00a0 Key Accountabilities: Conducts telephonic outreach to potential PCORI 3D study participants\/members and\/or caregiver to provide an overview of the research study and gain initial consent for comprehensive screening by UConn Center on Aging Research Assistants. Completes accurate and timely referrals of appropriate PCORI 3D study participants\/members and\/or caregiver to the UConn Center on Aging Research Assistants. Maintains critical documentation in the designated care management electronic medical record system, meeting defined timeframes and performance standards (e.g., tracking of outreach efforts and outcomes). Transmits weekly PCORI 3D study member census report to the UConn Center on Aging staff to notify them of enrolled study participants who have been admitted to a facility, have been hospitalized, or who have died. Working collaboratively with Medical Analytics team and Research Assistants, schedules outreach activities to MA members and documents activities identified for the PCORI 3D program Refers members that are identified as high risk, who decline the PCORI 3D program, to CCI\u2019s Telephone-based Care Management. Manages a flexible work schedule to meet the needs of the potential study participants and to optimize successful outreach and recruitment Assist in writing project reports Performs other related projects and duties as assigned. Required Skills Technical Knowledge and Experience: Experience recruiting for and conducting interviews with potential research participants, using an approved script Experience working with older adults, including those with physical, mental health, cognitive, and intellectual disabilities and their families Excellent oral and written communication, organizational, and interpersonal skills required. Associate\u2019s degree or an equivalent combination of education and experience in health promotion, health coaching, and health outreach and\/or wellness education. At least 1 - 3 years of experience in a managed care setting with familiarity with concepts of care management. Strong knowledge of medical terminology preferred. Ability to establish priorities and work independently. Previous system user experience in a highly automated environment and strong personal computer literacy on Windows products required.
